Chronic Disease Services provided by the Madison County Health Department include: Blood Pressure screenings, and injectable medication such as, Vitamin B12, Estrogen, and Testosterone.

​

For injectable medications you must have a physicians order instructing how the medication is to be administered and you are responsible for providing your own medication. The order expires one year from the date it was written, and it is the clients’ responsibility to obtain a new written order each year. Please call to determine what you will have to pay for this service. These services are provided Monday- Friday 7 AM- 11:30 AM, then 1 PM- 5 PM.

Blood pressure screenings are provided on a daily basis from 7:00 a.m. to 5:30 p.m. There is no charge for blood pressure screenings.

Self-Monitoring Blood Pressure Program

A month long program where you monitor your blood pressure at home twice daily. Monitoring your blood pressure daily could help with early diagnosis of many life threatening diseases. 

 

SMBP monitoring is the regular measurement of blood pressure by the client outside the clinical setting. SMBP requires the use of a home blood pressure cuff, that is supplied by LPHA at the time of enrollment. Client measures their  blood pressure at home in the morning and evening, twice each time. Enrollment is in person, Client will receive a loaner BP cuff kit along with logs and learning material. Client checks in once weekly for 3 weeks to calculate blood pressure averages. Follow up 1 and 2 can be done over the phone or in person. Week 3 follow up (Final visit) is an in person meeting where blood pressure cuff kit retuned.
