Chronic Disease Services provided by the Madison County Health Department include: Blood Pressure screenings, and injectable medication such as, Vitamin B12, Estrogen, and Testosterone.

​

For injectable medications you must have a physicians order instructing how the medication is to be administered and you are responsible for providing your own medication. The order expires one year from the date it was written, and it is the clients’ responsibility to obtain a new written order each year. Please call to determine what you will have to pay for this service. These services are provided Monday- Friday 7 AM- 11:30 AM, then 1 PM- 5 PM.

Blood pressure screenings are provided on a daily basis from 7:00 a.m. to 5:30 p.m. There is no charge for blood pressure screenings.

Living a Healthy Life (CDSMP)

​

A Chronic Disease Self-Management course ideal for people with ongoing health conditions like arthritis, heart disease, depression, and diabetes. The class meets for 2.5 hour sessions once a week for six weeks. You will learn skills to help you have increased energy & physical activity; decreased pain, fatigue, depression, & health distress; and fewer social limitations. Living a Healthy Life with Diabetes (DSMP)

​

A Diabetes Self-Management course for people with Type 2 Diabetes. The class meets for 2.5 hour sessions once a week for six weeks. You will gain tools to manage diabetes including developing action plans and learning ways to balance and control blood sugar, read food labels, and how to communicate with Healthcare Providers.
